Job Summary: We are seeking a friendly and compassionate Nurse Practitioner/ Physician Assistant to join our growing team. In this role, you will provide comprehensive pulmonary care to patients in post-acute care settings while collaborating closely with physicians, nursing staff, and interdisciplinary care teams. Responsibilities:
- Provide primary care services to residents in post-acute care settings
- Evaluate, treat, and manage patients experiencing acute or worsening medical conditions requiring timely intervention
- Perform physical exams and medical history assessments
- Prescribe medications and monitor treatment effectiveness
- Develop and implement individualized treatment plans
- Collaborate with interdisciplinary teams
- Educate patients, families, and facility staff on respiratory care and treatment plans
- Monitor patient progress and adjust care plans as needed
- Maintain accurate and timely EMR documentation
- Support initiatives focused on reducing hospitalizations and Return-to-Hospital (RTH) events
Required Qualifications:
- Preferred a minimum of 6 months of experience in post-acute care setting
- Preferred experience: training provided
- Active and unrestricted state Nurse Practitioner license or Active PA license and NCCPA certification required
- Master’s Degree in Nursing from an accredited Nurse Practitioner program or Master’s Degree from an accredited Physician Assistant program required.
- CPR BCLS/BLS certified
- Strong communication, organizational, and teamwork skills
- Ability to work independently in the clinical environment
