Responsibilities:

Oversees medication distribution as stated on physician orders, ensuring strict adherence to Community medication management documentation and procedure, and demonstrating steadfast dedication to resident safety

Upon thorough assessment, creates personalized service plans tailored to individual care needs while championing resident independence and dignity

Cultivates communication between physicians, healthcare providers, and family, fostering an effective exchange of communication pertaining to the physical, mental, and social well-being of our residents

Contributes toward quality assurance and performance improvement initiatives

Engages in continuous learning and development opportunities through attendance and participation in educational seminars, staff meetings, and other learning experience
Qualifications:

Current state licensure as Licensed Practical Nurse (LPN)

Current CPR certification

Exceptional communication and interpersonal skills

Must demonstrate the ability to serve with love, compassion, and dignity
