Job Summary
The Case Manager participates in outreach activities to identify and enroll SSVF-eligible veterans in case management services, ensuring timely access to safe, permanent housing and essential supportive services. The position utilizes a Housing First Model to develop and implement Housing Stability Plans that promote household stability and self-sufficiency.
Key Responsibiliities

Conduct full assessments with applicants to enroll SSVF-eligible veterans in case management for a minimum of 90 days, documenting all activities and events

Ensure rapid entrance into permanent safe housing or maintain stable housing to prevent homelessness

Mediate, advocate, and leverage support for eligible veterans, referring them to healthcare navigation, legal, employment, financial, and other services as needed

Identify and connect veterans to community resources for overall well-being, including emergency housing needs

Maintain accurate records of all resources provided, including temporary financial assistance, and manage client files safely while preserving confidentiality

Represent Baker Ripley and the Veteran Services team professionally in internal and external meetings and events

Collaborate with other agencies to provide a comprehensive case management process

Perform other duties as assigned
Education
Bachelor’s degree preferred; high school diploma or GED required
Experience
2+ years of case management experience; 1–3 years minimum; 2+ years social services experience preferred
Skills
Proficiency in Excel, clear and accurate correspondence/reporting, ability to work in a professional, client-focused environment
Licenses/Certifications
Valid driver’s license; experience working with veterans or in similar outreach roles is a plus
Knowledge, Skills, and Abilities
- Ability to develop Excel spreadsheets using formulas
- Ability to write clear and accurate correspondence and reports
- Ability to effectively present information and respond to questions both orally and in writing in a manner that is understandable to others
- Ability to calculate figures and amounts
- Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form
- Excellent interpersonal skills and successful experience working in teams
