A Brief Overview
Responsible for providing case management for children under the age of three with intellectual or developmental disabilities or delays. Ensures quality services are provided in accordance to all agency or government regulations.
What would you do in this position?
- Complete required paperwork and maintain accurate records in department database

Communicate professionally and positively with others

Schedule, attend, and facilitate meetings as required by the department

Maintain ongoing contact with family and providers, taking action as necessary

Coordinate, monitor, and change services

Assist with the transfer of services
-
Coordinate referrals and follow-up as needed

Find and secure funding for services

Maintain productivity standards as required by agency or government regulations
What education or experience would you need?
-
Valid Drivers License
-
High school diploma or equivalent required
-
Bachelor's Degree in a Human Services field preferred or 5 years or more experience working with people with disabilities required

Combination of education and experience may be used to meet the education and work experience requirements
What knowledge, skills, or abilities would you need?

Ability to communicate clearly in oral and written forms

Ability to use office equipment such as fax machine, scanner, copier, phone

Ability to work as a team

Analytical thinking skills to solve routine problems using common sense

Basic knowledge in Microsoft Word, Excel, and Outlook

Basic knowledge of state regulations

Basic knowledge of technology such as internet browsers, PDF software, database systems
