Overview
hospice community day position 8a-4p, every other weekend, one summer one winter holiday. Comp days during the week when working the weekend.
Responsibilities
- Deliver compassionate patient care by assessing symptoms, managing pain, and providing emotional support in accordance with hospice protocols
- Collaborate effectively with physicians, social workers, chaplains, and other healthcare team members to develop and implement personalized care plans
- Conduct thorough patient assessments including vital signs, physiological evaluations, and medical history reviews to inform treatment strategies
- Manage electronic health records (EHR) efficiently to document patient progress, medication administration, and clinical notes in compliance with HIPAA regulations
- Coordinate discharge planning and post-acute care services to ensure seamless transitions for patients returning home or to other care facilities
- Educate patients and families on disease progression, symptom management, and available support resources
- Participate in ongoing training related to infectious disease care, emergency response procedures, and health regulation policies to maintain high standards of practice
Qualifications
- Valid RN license with current registration in the state of practice
- Proven experience in hospice or palliative care settings with strong patient service skills
- Critical care experience including ICU or emergency medicine strongly preferred
- Experience working within Level I Trauma Centers or Level II trauma centers is advantageous
- Knowledge of hospital medicine, medical-surgical units, behavioral health, geriatrics, and home health environments
- Familiarity with EMR/EHR systems and medical terminology for accurate documentation
- Ability to perform comprehensive patient assessments and manage complex clinical cases effectively
- Strong understanding of HIPAA regulations and health policy compliance
- Excellent communication skills for client interaction and interdisciplinary collaboration
- Capable of managing acute care situations with professionalism and empathy

Pay: $69.00 - $70.00 per hour
