Clinical Supervisor RN, Home Health

Center of Illinois Home Healthcare, Inc. is hiring an experienced Clinical Supervisor RN for our Medicare-certified, ACHC-accredited home health agency.
We are seeking a strong clinical leader with home health experience, Medicare knowledge, and OASIS understanding to help oversee daily clinical operations and support our growing team.
This position works closely with nursing, therapy, scheduling, intake, QA, and leadership teams to ensure quality patient care and regulatory compliance.
Responsibilities:
- Supervise and support field clinicians including RNs, PTs, OTs, SLPs, and home health aides
- Review OASIS documentation and Plans of Care for accuracy and Medicare compliance
- Ensure compliance with CMS, ACHC, and agency policies
- Assist with clinician education, training, and performance management
- Monitor clinician productivity and documentation timeliness
- Participate in case conferences and interdisciplinary meetings
- Collaborate with intake, scheduling, QA, and leadership teams
- Assist with patient care coordination and clinical problem-solving
- Support quality assurance and performance improvement initiatives
- Maintain strong communication with physicians, referral sources, and clinical staff
Qualifications:
- Active Illinois RN license required
- Minimum 2 years of Home Health experience required
- Previous Clinical Supervisor or leadership experience preferred
- Strong knowledge of Medicare Home Health regulations
- OASIS experience required
- Axxess EMR experience preferred
- Strong organizational and communication skills
- Ability to multitask in a fast-paced healthcare environment

Schedule:
- Monday to Friday
- On-call rotation as needed
Experience:
- Home Health: 2 years required
- OASIS: 1 year preferred
- Clinical leadership: 1 year preferred
License/Certification:
- Illinois RN License required
