Position Title: Qualified Mental Health Professional

Position Description:
Under the supervision of the Clinical Director, the "Qualified Mental Health Professional-Adult" or "QMHP-A"will provide direct Mental Health Support Services (MHSS) that implement a service plan, which provides structure, support, and services to client’s. These services are provided in accordance with agency policies.

Major Duties:
Position Responsibilities:
1. Provides direct, Mental Health Support Services (MHSS) to identified client (Ensure that the required LMHP documents have been gathered and completed to ensure that the
Qualified Mental Health Professional-Adult (QMHP-A)and all involved parties can participate in the preparation of treatment plan and updates.
2. Assists in planning of services.
3. Provides client with day to day skills/education as well as support to assist client in meeting their day to day obligations.
4. Models and teaches problem-solving techniques.
5. Provides a variety of interventions in client’s home and community as appropriate and necessary. These interventions may be behavioral, financial and/or from a mental health skill building
6. Provides linkage to community-based services and supports; provides advocacy and support for the client.
7. Coordinates scheduling of appointments, as needed.
8. Coordinates services, as needed. Develop a crisis plan and Link to appropriate mental health provider
9. Is responsible for the proper and timely documentation of all information required by the
Department of Behavioral Health & Developmental Services (DBHDS) and P.A.C
Mental Health Services, LLC.
10. Maintains regular and ongoing contact with referring agencies, as well as other agencies involved in client
11. Actively coordinates and plans discharge, and aftercare plans with referring agency.
12. Attends interagency meetings, as necessary.
13. Attends and participates in team meetings.
14. Attends and participates in orientation, in-service training, and conferences as scheduled.
15. Attends supervision.
16. Performs other duties as assigned.
17. Driver's License w/current inspection and vehicle auto insurance acceptable driving record
Attend and participate in new employee orientation, in-service training, conferences as scheduled and performs other duties as assigned. Receive ongoing training throughout the year, maintain and keep current all first aid and CPR training requirements, "Qualified Mental Health
Professional-Adult" or "QMHP-A"staff will be trained in Therapeutic Options (behavioral intervention training). All applicants must complete an employment application, have a valid
Driver's License w/acceptable driving record, undergo and pass a background check, and provide references.
Knowledge, Skills, and Abilities
- Knowledge of principles and techniques of basic assessment skills;
- Working knowledge of human services system, roles of human services personnel, and relevant community resources;
- Ability to maintain effective working relationships with other social services providers;
- Ability to communicate effectively both orally and in writing.

Qualifications/Requirements (PLEASE READ): If all qualifications are not met, your application will not be considered.
Must be Certified with the Department of Health (DHP).
1. A master's degree in psychology, social work, counseling, substance abuse, or marriage and family therapy verified by an official transcript from an accredited college or university;
2. A master's or bachelor's degree in human services or a related field verified by an official transcript from an accredited college.

Traveling required at least 85% of the time within the Hampton Roads Area; must use own vehicle and provide proof of automotive/liability insurance.
