Position Summary:
Acclaim Home Health is seeking a compassionate and motivated Occupational Therapist (OT) to join our growing home health team in Worcester, Massachusetts. This is an excellent opportunity for a skilled clinician who is passionate about helping patients regain independence and improve their quality of life in the comfort of their own homes.
As a Home Health Occupational Therapist, you will work one-on-one with patients to evaluate functional needs, develop personalized treatment plans, and provide therapeutic interventions that support daily living and mobility.
Responsibilities
- Evaluate patients’ functional status and occupational therapy needs in the home setting
- Develop individualized treatment plans and goals
- Provide therapeutic exercises and adaptive training
- Educate patients and caregivers on safety, mobility, and activities of daily living
- Recommend adaptive equipment and home modifications
- Collaborate with physicians, nurses, physical therapists, speech therapists, and caregivers
- Complete accurate and timely clinical documentation
- Monitor patient progress and adjust care plans as needed
Qualifications
- Current Massachusetts Occupational Therapist (OT) license
- Graduate of an accredited Occupational Therapy program
- Home health experience preferred, but not required
- Strong communication and interpersonal skills
- Valid driver’s license and reliable transportation
- Ability to work independently and manage time effectively
