JOB
The Outreach Behavioral Health Case Manager coordinates the overall stabilization plan for assigned caseload of clients referred to the Outreach Behavioral Health Team. This position is responsible for identifying treatment needs; developing, and monitoring stabilization plans and progress; and making referrals to necessary interdisciplinary providers and community partners.
Care Coordination will include collaboration with existing services available through Island County and all other service providers in the county including mental health and medical needs, housing assistance, referrals to advocacy groups, schools, community liaisons and other needs as identified through assessments.
This position will serve those who have extensive history of psychiatric hospitalizations, may be unhoused, have co-occurring substance use disorders, medical problems, or are involved in the criminal justice system. Services might include helping individuals with housing, parenting, benefits, symptom management, medical care, substance use disorder treatment and employment.

EXAMPLE OF DUTIES
- Provides community outreach and early intervention case management services to individuals and/or families who are at risk of a behavioral health crisis or in need of intensive care coordination. Completes situational assessment and provides immediate support to all clients entering caseload.

Develops recovery-oriented stabilization plan with clients and/or their families as the means for implementing appropriate services. Responsible for coordinating services being provided; arranging resources, advocating for needed services, intervening in crises, and making appropriate referrals.

Develops and maintain a working relationship with community providers which may include crisis stabilization facilities, crisis responders, behavioral health treatment providers, health care providers, shelter providers, landlords, and other community programs which may support participants.

Documents all contacts in a timely basis including face-to-face interviews, collateral and networking contacts, and correspondence. Maintains case records in accordance with standards and requirements. Prepares reports by collecting, analyzing, and summarizing treatment and results data.

Prepares case closure in a timely manner by reviewing and communicating care plans, orienting family members and other collateral contacts and providing resources. Provides follow up services when appropriate and necessary.

Communicates clients' progress by attending weekly interdisciplinary meetings and supervision in compliance with agency policies and procedures. Collaborates and works with other community agencies.

Updates job knowledge by participating in educational opportunities, reading professional publications, maintaining personal networks, and participating in professional organizations.

Follows policies, procedures, and protocols for this program as a member of the Outreach Behavioral Health Team.

Performs other duties as assigned by the supervisor which are consistent with the position.
SUPPLEMENTAL INFORMATION
- Bachelor’s degree in Social Work, Psychology, Sociology, Human Services, or related field required.

Two or more years of experience in providing crisis mental health and/or case management services required

Prior relevant experience and training may be substituted for years of experience
