Purpose: We are seeking a full-time Nurse Practitioner to join our Telehealth Psychiatric Health Care delivery team as a fully remote clinician, providing medical care via telemedicine to patients residing in skilled nursing facilities. The Telehealth Psychiatric Mental Health Nurse Practitioner will work as part of the Collaborative Care Model (CoCM) Program and will be responsible for collaborating with an interdisciplinary team to support a population-based approach to mental health care, improve clinical outcomes, and ensure continuity of care across settings.

Hours: Monday – Friday day shift hours
Role & Core Responsibilities:
- Provide evidence-based psychiatric care, remotely, to clients in long-term care (LTC) facilities.
- Evaluate, diagnose, prescribe, and provide treatment recommendations.
- Daily work with telehealth facilitators who will manage your virtual platform through tablet to provide care for the patient and interact with facility staff.
- Adhere to federal and state regulations specific to LTC facilities, especially as it relates to the diagnosis of justification for antipsychotics and gradual dose reduction (GDR).
- Meet with the Director of Nursing (DON) and other facility staff to communicate care recommendations and meet any local needs.
- Complete necessary facility forms and other required documentation.
- Provide support during daytime hours to After Hour Telehealth provider team issues as it relates to mental health care.
- Work with a therapy team to address behavioral needs.
- Participate in Interdisciplinary Care Team meetings.
- Complete orders daily; notes and billing within 48 hours.
- Monitor patient progress using standardized behavioral health screening tools
- Actively participate in theCollaborative Care Model (CoCM) by collaborating with interdisciplinary teams
- Engage in regular case reviews and support population-based patient management
- Coordinate care across the interdisciplinary team and facilitate referrals as needed
- Educate patients and support treatment adherence
- Maintain accurate documentation and ensure compliance with telehealth and CoCM billing requirements
Credentials:
- Active and unrestricted state licensure in a minimum of five states and willingness to obtain additional if requested.
- Active and unrestricted Advanced Practice Nurse, National Board certification (AANP, ANCC or AACN) board certification.
- Psychiatric Mental Health Certification is required.
- Active DEA licensure, or willingness to obtain in the states of practice is required upon hire.
- Must have an active national provider number (NPI).
- Must have active Medicare & Medicaid provider numbers, or willingness to obtain them upon hire.
- Must be eligible to participate in the Medicare & Medicaid programs as a provider.
Qualifications:
- A minimum of three years' experience as a Psychiatric Mental Health Nurse Practitioner is preferred.
- Long-term care experience is preferred.
- Telehealth mental health experience is required.
- Experience managing patients with polypharmacy, completing gradual dose reductions (GDRs) and addressing deprescribing opportunities is required.
- Technology proficiency is required.
- Excellent people skills: communicating with patients, families, and members of the healthcare team.
- Self-directed with the ability to practice autonomously, but functions as a member of the team.
- Excellent clinical skills with the ability to assess, diagnose, develop a treatment plan, and communicate in a virtual environment.
Compensation:
$60-$80 per hour (within the range, geography, individual pay is determined by job-related skills, experience, licensure, and relevant education or training).

Working conditions:
A designated, secure, and private home workspace is required for this remote role to ensure compliance with HIPAA and company privacy and security policies. This workspace must support confidential patient communications and prevent unauthorized access to protected health information. Typing is required and you will be required to use Company approved virtual technology to visualize patients during visits. A personally owned computer used for this role must meet all company-mandated security and encryption requirements as defined by the IT team. Company-provided security software, including platforms for patient video consultations, may need to be installed to ensure the device remains compliant with organizational data protection standards. Periods of prolonged sitting or standing can be expected. Travel requirements could occur based on specific state licensure and leadership needs.
