Ultimate Therapy is seeking a Certified Occupational Therapy Assistant (COTA) to join the rehabilitation team at Autumn Lake at Norwalk. This role is a great opportunity to work directly with residents while supporting their ability to complete everyday activities with greater confidence and independence.

Provide occupational therapy treatments under the supervision of a licensed Occupational Therapist

Assist residents with activities of daily living, strengthening, coordination, balance, and functional tasks

Follow individualized treatment plans and support resident-specific rehabilitation goals

Observe progress and communicate treatment updates to the supervising OT

Reinforce adaptive techniques and safety strategies with residents and caregivers

Maintain accurate and timely clinical documentation

Work collaboratively with therapy, nursing, and other members of the care team
