An exciting opportunity is available for an experienced and compassionate Registered Nurse with expertise in home health. This contract role is essential in delivering high-quality, individualized nursing care to patients within their homes, ensuring both clinical excellence and patient comfort.
Schedule:
- Monday – Friday, 8:00 AM – 5:00 PM
Key Responsibilities:
- Complete OASIS-based comprehensive patient assessments and establish tailored care plans.
- Deliver skilled nursing interventions including wound care, IV therapy, chronic disease management, and medication administration.
- Track patient progress and promptly communicate any status changes to physicians and the care team.
- Provide patient and family education focused on disease management, medications, and preventive health.
- Maintain accurate, timely notes using Home Care Home Base (HCHB) electronic documentation.
- Collaborate closely with multidisciplinary professionals to ensure cohesive care delivery.
- Attend regular case conferences, team meetings, and engage in continuing education.
- Adhere strictly to regulatory and compliance standards for home health services.
Qualifications:
- Active RN license (in good standing) in the state of practice.
- Minimum of 1 year of recent home health field experience.
- Proficiency in Home Care Home Base (HCHB) and completion of OASIS assessments.
- Demonstrated clinical assessment, critical thinking, and time management skills.
- Reliable transportation, valid driver’s license, and current auto insurance.
- Ability to independently manage a dynamic, field-based caseload (3–5 visits daily).
- Strong communication and organizational abilities.
- BSN preferred and prior experience managing a caseload of 25–32 points per week highly desirable.
- Familiarity with Medicare and home health compliance standards is a plus.
