(based on experienceshift differentials)
Per Diem: $27-$31/hour
(based on experience and shift differentials)
Pick-Up Pay: +$5/hour weekdays | +$7/hour weekends

What You'll Do as an LPN:
- Safely provide medications and treatments as prescribed by physicians, following accepted standards of practice and organizational policies.

Maintain accurate, timely written and electronic records in compliance with legal, regulatory, and company requirements.

Monitor and document residents' conditions, report changes to the RN and relevant team members, and collaborate in developing and implementing individualized care plans.

Assist with admissions and readmissions, gather clinical data, support resident care programs by ensuring adherence to plans of care, and oversee CNA performance in these areas.
LPN Requirements:

Active Wisconsin Licensed Practical Nurse (LPN) license

Current CPR certification
- Long-term care experience preferred
- Basic computer skills

Ability to push/pull up to 60 lbs. and occasionally lift up to 100 lbs.
