Group Home Location:

- Hybrid schedules or working from home are not available options. The employee’s physical presence in the home is a crucial service we provide to our consumers.
Work Schedule:
- This is a full-time position.
o Applicants must be flexible to work afternoon shifts, and other shifts as required including holidays and weekends.
o Daytime shifts are 7am - 3pm
o Afternoon shifts are 3pm - 11pm
Compensation:

Performs direct care. Documents timely on all hours worked with participants in the progress notes.

Reviews progress notes daily for accuracy and thoroughness. Documents and follows up on any issues.
-
Responsible for documentation of individual plan of service and offers input for changes.
-
Ensures guardian signatures are on all applicable documents.
-
Works in consultation with funding agency to maintain current plans of service, annual assessment, and nursing assessments and PPV’s as applicable.
-
Completes Transition for the Future on a yearly basis in accordance with the PCP or as clinically needed.
-
Completes Incident Report checklist.
-
Ensures participant Individual Record Index documents are kept up to date and uploaded after completion into the electronic medical record.
-
Oversees completion of participant inventory.
-
Carries cell phone for emergencies and is available on-call 24 hours as needed.
-
Licensing Assessment and Agreement of Service completed and updated annually, if applicable.
-
Menu’s reviewed for accuracy with PCP and physician ordered diets, if applicable.

Follows MI mental health code.
-
Supervises all staff on all shifts, completes monthly staff observations and disciplines as needed.
-
Evaluates in person, within 2 weeks of due date of evaluation, giving constructive feedback for improvement.

Holds staff meetings one time a month with minutes. Based on ALS-LM agenda.

Ensures there are adequate staff levels to meet the needs of consumers and
-
Keeps up-to date staff availability forms and updates as needed.
-
Completes worker compensation reports as needed and Annual MIOSHA forms.
-
Keeps staff files up to date following the arrangement of employee files form.
-
Completes annual self-appraisal 2 weeks prior to evaluation due date.

Completes quarterly data collection in accordance with Quality Improvement Plan: Community Integration, Hospitalization

Ensures status changes in E.M.R. are updated as needed.
-
Communicates in a positive manner with staff.
-
Ensures all program audit and licensing standards are maintained and remediation’s completed timely when applicable.
-
Ensures monthly participant meetings are completed and documented.
-
Ensures all seasonal maintenance and physical plant issues are addressed and completed timely.
-
Ensures H & S Audit standards are maintained and remediation’s completed timely.
-
Follows all H & S/OSHA guidelines.
-
Makes sound fiscal decisions with fiscal department input (stays within +0%, -10% of budget)

Monitors and ensures service authorizations are current.
-
Reconciles and balances monthly food certificate log/food stamp logs, emergency funds/ all resident funds forms and petty cash.
-
Applies as needed for participant entitlements upon admission, discharge and changes and notifies outside agencies as applicable.

Works to improve site by obtaining donations, volunteers and other benefits for the site and for the corporation
-
Stays within 5-10% of site contract hours based on site occupancy.

Keeps track of site inventory.
-
Completes daily punches, and/or pre-populated schedule.
-
Completes Annual training required to include; Contract Agency requirements, on-line training (if applicable), in-home quarterly training.

Checks email daily during scheduled shifts and respond within 1 business day.
-
Monitor staff time & attendance daily throughout the pay period.
-
Develops staff schedules at least two weeks in advance and records changes as needed. Uploads into electronic payroll system prior to schedule beginning.

Completes timecard reconciliation weekly.
-
Trains new staff on E.M.R. role and tasks.
-
Trains staff and completes on-site training checklist in a timely manner.
-
Trains staff in person centered plan/behavior plans/site in-services.
-
Schedules all staff required training and re-training timely.
- Demonstrates thorough knowledge of ALS philosophy and policies and procedures or has equivalent experience with another similar setting. Demonstrates leadership and problem-solving skills

B.A., B.S. or equivalent of 2 years experience in the field of Human Services or related fields and one year supervisory experience.

Must be able to lift 50 lbs or physically position individuals (with training). This is an essential job function that may involve up to 20% of scheduled time.

Good written skills/oral skills.
-
CPR/FA training.
-
Negative on TB test.

Clear criminal record.

Valid driver's license/car.

Ability to be on call
