We're looking for a detail-oriented, dependable, and compassionate Clinical Operations & Medication Coordinator to help ensure our residents receive safe, accurate, and well-coordinated care behind the scenes. Key Responsibilities:
Medication Management
- Review physician orders and process medication changes
- Update medication records and resident Cardex
- Coordinate with the pharmacy regarding medication orders
- Remove discontinued medications from medication packs (per facility policy)
- Maintain accurate medication documentation
- Track physician orders from receipt through completion to ensure no follow-up is missed.
Clinical Coordination
- Review physician visit documentation and process new orders
- Communicate with physicians when clarification is needed
- Coordinate weekly laboratory requests and track laboratory results
- Follow up on physician orders and laboratory findings
- Assist with the medical portion of resident admissions
Documentation & Administrative Support
- Maintain resident clinical records and documentation
- Process hospital and emergency room return paperwork
- Support compliance documentation and regulatory requirements
- Perform other related duties as assigned by the Administrator.
Qualifications:
- Connecticut Medication Certification required
- Ability to read, understand, and accurately implement physician orders
- Experience in assisted living, residential care, or long-term care preferred
- Strong organizational and documentation skills
- Excellent communication and attention to detail
- Comfortable working with electronic records and computer systems
- Knowledge of pharmacy coordination and state regulations is a plus
- Dependable, professional, and team-oriented
