JOB DESCRIPTION – Ogemaw Commission on Aging
Supervisor: In Home Service Coordinator
Special considerations:
· Requires minimum 2 weeks’ notice of resignation (see Handbook for details)
· Requires pre-employment physical
- Requires use of personal vehicle and ongoing proof of current insurance
QUALIFICATIONS:
- Must be a mature individual able to relate to the senior population.
- Must be of a confidential nature.
- Experience in home health care or related field.
- Must be capable of lifting/supporting 50 pounds.
- Reliable transportation, valid driver’s license and proof of insurance required.
DUTIES:
- Receive participant names and care requirements from the In Home Service Coordinator and schedule services as ordered.
- Provide light housekeeping as ordered; including vacuuming, dusting, floors, bathrooms, laundry, ironing, meal preparation, etc.
- Provide personal care as ordered; including bathing, showering, bed bath, toileting, transferring, assisting with movement, etc.
- Provide respite care as ordered; including relief for caregivers while meeting homemaking and/or personal care needs and socialization for the participant
- Keep Supervisor(s) informed of current or potential problems regarding participants.
- Maintain accurate documentation regarding hours per participant, units utilized, mileage and services provided and submit to office as required.
- Wear COA identification at all times when in participant home
- Attend all necessary trainings and meetings as required.
- Abide by all State, Federal, AASA, and COA standards.
- Assume any additional duties as assigned by the In Home Service Coordinator.
PHYSICAL DEMANDS – pre-employment physical required
· Work 5-7 hours/day, mostly standing/moving
· Ability to grip/grasp - throughout day
· Ability to bend, twist, reach (some over-head), walk, stand - throughout day
· Pushing/pulling - throughout day
· Ability to lift/transfer up to 50 pounds - occasionally, or with assistance
· Vision sufficient to read paperwork
· Hearing sufficient to communicate with team/participant
WORKING ENVIRONMENT
· Indoor – in participant’s home
· Driving
· Hazards include: stove, knives, other home equipment
Pay: $13.73 - $14.75 per hour
