JOB Under the direction of the Director of Health Education, Cultural and Linguistic Services, and Disease Management (Director of Health Education) this person will assist in the implementation and coordination of a Health Navigator Program. The Health Navigator services may include, but not limited to, guiding members through the healthcare system, help members establish a “medical home” and assist members in reducing barriers to obtaining their health care needs. The Health Navigator will establish contact with members who utilize the emergency room and assist members in reconnecting with their primary care provide after an emergency room encounter. EXAMPLE OF DUTIES
- The Navigator will contact designated members after ER discharge and assist members in following-up with their Primary Care Provider (PCP) and other medical appointments.
- The Navigator will assist members with health education services within health plan as well as in the community health education resources.
- The Navigator will assist members with access to needed services and in educating members about the concept of a medical home. The Navigator will also refer members in need of care management to the health services department.
- The Navigator will maintain a tracking and evaluation system of all contacts and follow-up.
- The Navigator will work with the member to increase awareness and education about how to use their PCP and when to go the ER.
- The Navigator will work hospital staff, plan partners, and community health clinics to help educate and assist members with their needs.
- The Navigator will assist members in scheduling follow-up appointments after discharge, assist with filling prescriptions, and help to reduce barriers or challenges to meeting their healthcare needs.
- The Navigator will provide telephonic assistance as needed.
- The Navigator will work with hospital and clinic personnel to educate them about the Health Navigator Program and goals of the program.
- The Navigator will prepare reports and preform others duties as needed.
