Job Overview
We are seeking a dedicated and highly skilled Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong foundation in patient care, with experience across various clinical settings including long-term care, assisted living, hospitals, and specialized care units. Duties
- Perform comprehensive patient assessments by monitoring vital signs, conducting physical examinations, and evaluating patient conditions to inform care plans.
- Administer medications and injections accurately, including IV infusions and catheter care, following established protocols and safety guidelines.
- Assist with diagnostic evaluations such as specimen collection, processing, and supporting medical imaging procedures.
- Provide specialized care for patients with complex needs including Alzheimer's disease, dementia, memory care, and behavioral health management.
- Support emergency response efforts through first aid, airway management, CPR certification, and acute pain management techniques.
- Maintain detailed medical records using electronic health record (EHR) systems such as Epic or Cerner while ensuring compliance with HIPAA regulations.
- Collaborate effectively with physicians, case managers, caregivers, and families to develop personalized care plans aligned with long-term care regulations and best practices.
Experience
- Proven experience working as an Licensed Practical Nurse in diverse clinical environments such as hospitals (including ICU, trauma centers), nursing homes, or outpatient clinics.
- Demonstrated proficiency in using EMR/EHR systems like Epic, Athenahealth, or eClinicalWorks for documentation and patient management.
- Critical care experience including ICU or Level I Trauma Center settings is highly desirable.
- Familiarity with ICD coding (ICD-9/ICD-10), CPT coding for procedures, and medical terminology essential for accurate documentation.
- Experience working with individuals with disabilities or developmental delays; knowledge of dementia and Alzheimer’s care preferred.
- Additional experience in hospice care, home health services, or pediatric settings such as NICU or PICU is advantageous.
- Knowledge of infection control practices, sterile techniques such as suturing or phlebotomy, and advanced skills like ventilator management or spinal taps will strengthen candidacy.
