Job Summary
We are seeking a passionate and dedicated Community Service Program (CSP) Provider to make a meaningful difference in the lives of adults and families. In this vital role, you will provide client advocacy, ensure continuity of care, support clients in time of crisis, provide/procure skills training, ensure the acquisition of necessary resources, and assist clients in achieving community/social integration. Your energy and commitment will help empower adults and families to overcome challenges, navigate complex systems, and build healthier, more resilient futures. This position offers an exciting opportunity to work directly with adults and families in a dynamic, supportive environment. Bilingual is definately a plus.
Responsibilities:
- Assist in the coordination of services and case management activities including coordination or assistance in accessing medical, social, education, housing, transportation, or other appropriate support services as well as linkage to more/less intensive community services
- Conduct comprehensive, strengths-based assessments to develop individual Service Plans with each client on an on-going basis. Regularly assess client treatment plans, cand/or family needs.
- Provide client support and interventions with focusing on daily living, education, budgeting, medication compliance, relapse prevention, social skills, and independent living skills.
- Advocate for clients' rights and assist clients in ensuring that their rights are protected and preserved.
- Provide or arrange transportation as needed to meet treatment plan goals and other related activities.
- Ensure progress notes are completed within NLCWC’s mandated period.
- Meet productivity benchmarks as assigned by NLCWC’s productivity plan
- Participate fully in all individual and group supervision meetings.
- Submit work schedule to administrative staff weekly.
- Attend internal supervision and mandated trainings scheduled by NLCWC, remain current in the field to pursue professional growth and development.
- Establish collaborations with providers, state agencies, schools and other community and natural supports.
- Adhere to NLCWC’s expectations and policy for Telecommuting & Telehealth services.
- Will require working nights and weekends.
- Other duties as needed.
Skills:
- Experience working with adults and families.
- Knowledge about family engagement practices, family dynamics and assessment and community resources; knowledge of the service delivery system such as DCF, DMH, DYS, DDS and the Court system
- Background in mental health, substance use recovery or homelessness support services
- Strong crisis management, interpersonal, and communication skills
- Demonstrated cultural competency in working with youth & families from different cultures and ethnicities.
- Must have a current Drivers’ License and a Reliable Vehicle since this position involves driving to see clients in person.
- Bilingual language capabilities a plus
- Demonstrated ability to work in a team environment.
- Excellent organizational and time management skills
Education and Experience:
- Minimum High School diploma with commensurate experience required.
- At least 12 months of prior work-related experience preferred.
Pay: $20.00 - $25.00 per hour
