The Admissions Coordinator plays a crucial role in growing and stabilizing the census and payer mix of our facilities through various strategies. This includes exercising a working understanding of the census and payer mix needs of the facilities, promoting a positive image of the organization, and establishing productive working relationships within the community.
Responsibilities:
Increase number of Referrals to generate Admissions
Demonstrate a clear and working understanding of the admissions criteria and goals of the facility
Identify primary and secondary referral sources for SNF and Assisted living and develop working relationships to increase referrals and admissions
Develop and maintain a collaborative and cooperative role with internal facility leadership and external referral sources
Assure seamless and customer-friendly care transitions between referral sources and facilities
Develop a routine outreach schedule for consistent messaging and familiarity of name in the community
Operations:
Identify, track, monitor, and trend admission and referral statistics using existing facility tools and software
Make formal reports to facility leadership and ownership regarding census, revenue, payer mix, referral source management, admissions, discharges, etc.
Develop and implement an effective tracking system for contacting and communication with key referral sources, case managers, and physicians
Professional Requirements:
Adhere to dress code and maintain a neat and clean appearance
Complete annual education requirements
Maintain regulatory requirements
Maintain resident confidentiality at all times
Report to work on time and as scheduled, complete work within designated time
Wear identification while on duty, use computerized punch time system correctly
