Duties
- Assess patient needs thoroughly by reviewing medical histories, conducting physical evaluations, and understanding physiology and anatomy fundamentals.
- Develop and implement individualized care plans that incorporate physiotherapy techniques such as trigger point therapy and orthopedics to address specific conditions.
- Deliver hands-on therapy sessions focusing on improving mobility, reducing pain, and restoring function across diverse settings including home health, outpatient clinics, hospitals, and community care environments.
- Coordinate discharge planning with healthcare teams to ensure seamless transitions from inpatient or hospital stays to home or community care.
- Educate patients and caregivers on treatment strategies, exercises, and safety measures to promote ongoing recovery and prevent future injuries.
- Document patient progress accurately using EMR (Electronic Medical Record) systems while maintaining compliance with healthcare regulations.
- Collaborate with physicians, nurses, occupational therapists, and other professionals to optimize patient outcomes through integrated care approaches.
Qualifications
- Valid licensure as a Physical Therapist (PT) in the respective state or region.
- Proven experience in home & community care settings with familiarity in outpatient, inpatient, hospital experience, or clinic environments.
- Strong knowledge of physiology, anatomy, medical terminology, and physiotherapy techniques including trigger point therapy and orthopedics.
- Prior experience with discharge planning and managing care for diverse populations such as pediatrics and geriatrics.
- Ability to utilize EMR systems efficiently for documentation and communication purposes.
- Excellent patient care skills with a compassionate approach tailored to individual needs.
