Job Summary
1. Assist patients while adhering to the patient’s plan of care provided by the Registered Nurse- Home Care (RNHC) and document on the patient’s daily activity record. All deviations from the approved care plan must be done with prior approval from the supervising nurse.
2. Effectively communicate with the care team when a patient appears to be out of sorts and something seems to be wrong immediately.
3. Utilize proper techniques for transfers, positioning, and assisting clients with walking to prevent injury and promote mobility.
4. Assist with Physical Therapy or Occupational Therapy assigned exercise activities per approved plan of care.
5. Assist patients with grocery shopping, errands, meal preparation, and light housekeeping as documented on the patient’s plan of care.
6. Provide transportation to medical appointments as needed. Accurately log mileage for transports and abide by all fleet policies and ordinances.
7. Assist RNHC with medication drop-offs for patients.
8. Provide companionship to the patients and respite for the patient’s family when able.
9. Follow weekly schedules as written by supervisor and notify supervising RNHC of any changes immediately.

Duties
- Assist residents with activities of daily living (ADLs) such as bathing, dressing, grooming, and toileting to promote comfort and independence
- Monitor patient health by observing vital signs, reporting changes, and maintaining accurate care records in compliance with HIPAA regulations
- Support medication administration and ensure adherence to care plans developed by nursing staff or healthcare professionals
- Prepare nutritious meals and assist with meal consumption while respecting dietary restrictions and preferences
- Facilitate patient transfer methods using equipment like Hoyer lifts or patient transfer techniques to ensure safety during mobility tasks
- Maintain a clean and safe environment through regular cleaning duties, including laundry, sanitation of common areas, and personal hygiene assistance
- Provide transportation assistance for appointments or community outings while ensuring safety protocols are followed
Qualifications
EDUCATION:
1. Must have a high school diploma or equivalent.
ESSENTIAL:
1. Must be 18 years of age or older.
2. Valid driver’s license, dependable transportation, and proper car insurance are required.
3. Must promote a healthy, non-abusive lifestyle.
4. Must be able to pass a pre-employment background check and will be subjected to periodic checks throughout employment.
5. Current First Aid/CPR certification or must be obtained within one (1) month of the hire date.
6. Complete National Incident Management System (NIMS)/Incident Command System (ICS) 100 and 700 certification within six (6) months of the hire date.
7. Personal Care Worker Certificate must be completed within six (6) months of hire date.

Pay: $17.15 - $24.92 per hour
