Supervising Registered Nurse (RN)
Pediatric Private Duty Home Care | Oakland, California
Location: Private residence, Oakland, CA
Schedule: Part-time / per diem — approximately 4–8 hours per month, scheduled by mutual agreement
Compensation: $75 per hour, including paid commute time
Reports to: Family- Patient’s mother, in coordination with the child’s primary care physician

About the Role
We are a family in Oakland seeking an experienced Registered Nurse to serve as the supervising RN for a small team of Licensed Vocational Nurses providing in-home skilled nursing care to our 9-year-old son, who has complex medical needs.
This is not a hands-on daily bedside role. The LVN team provides direct care; the supervising RN is responsible for building and maintaining the plan of care, validating LVN competency, and ensuring the care delivered at home matches what his physician has ordered. We are looking for someone who wants a steady, long-term relationship with our family.
Patient Overview
Our son is 9 years old and lives at home with his family. His care involves:
- respiratory care: oxygen therapy / administration and titration, pulse oximetry monitoring, suctioning, nebulizer and vest treatments
- G-tube feeding and medication administration
- seizure disorder and rescue medication protocol, including VNS therapy
- mobility limitations, positioning, and transfers
He attends school in OUSD and has an active care team including PCP, pulmonology, neurology, epileptology, GI, nutritionist, cardiology, ophthalmology, otolaryngology.
Key Responsibilities-Plan of Care Development and Maintenance
- Partner with the parents and the primary care physician to develop a comprehensive, written home care plan covering daily routines, medications, equipment, feeding, respiratory support, seizure and emergency protocols, and escalation criteria, with a focus on Palliative Care.
- Review and update the plan of care at least every quarter, and after any hospitalization, change in physician orders, or significant change in condition
- Translate physician orders into clear, actionable nursing instructions the LVN team can follow consistently
Supervision of LVN Staff
- Conduct quarterly in-home supervisory visits with each LVN to observe care delivery, confirm adherence to the plan of care, and verify competency with equipment and procedures. Must observe/meet with LVN during their shift.
- Complete and maintain written supervisory visit notes and competency checklists
- Provide coaching, correction, and skills training as needed; escalate performance concerns to the family promptly
- Orient and validate the competency of any new LVN joining the team before independent shifts
Clinical Oversight
- Perform periodic nursing assessments and communicate findings to the physician and family
- Review and help refine emergency preparedness plans, including power outage and backup equipment protocols and evacuation planning
- Serve as clinical liaison between the family, LVN team, primary care physician, specialists, DME vendors, and home health or insurance case managers as needed
Communication
- Provide the family with a brief written summary after each supervisory visit
- Be reachable by phone or secure message for clinical questions during agreed hours, and participate in care conferences as scheduled
Required Qualifications
- Current, active, unencumbered California RN license (BRN) in good standing
- Minimum 3 years of nursing experience, including at least 1 year in pediatrics
- Experience supervising or precepting LVNs, CNAs, or nursing students
- Current BLS certification; PALS strongly preferred
- Demonstrated competency with pediatric respiratory care / enteral feeding / seizure management
- Experience with and comfortable with Palliative care treatment
- Strong written documentation skills
- Valid California driver’s license, reliable transportation
- Willingness to complete a background check and to provide proof of required immunizations
- Comfortable working in a private home and respecting family routines and privacy
Preferred Qualifications
- Home health, private duty, or pediatric complex care experience
- Familiarity with California Title 22 home health regulations and LVN scope of practice
- Experience coordinating with Regional Center, California Children’s Services (CCS), or Medi-Cal waiver programs
Compensation and Terms
- $75 per hour, including documented commute time to and from the home
- Hours are variable and scheduled in advance; estimated 4–8 hours per month, with additional hours available during care plan revisions, hospital discharges, or onboarding of new LVNs.
- Onboarding month: approximately 8–12 hours to develop the initial plan of care and complete first competency validations
- Position classified as [W-2 household employee / independent contractor] background check/livescan to be completed and cleared before working with client
Work Environment
Private single-family residence in Oakland. The home is equipped with medical equipment and oxygen. There is a dog, 2 parents and 1 other child in the home. Non-smoking household. Parking is available on the street in front of the home.
