Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using manual handling techniques and equipment such as Hoyer lifts.
- Monitor patient health by checking vital signs, observing for changes in condition, and reporting findings accurately to healthcare teams.
- Prepare nutritious meals aligned with dietary restrictions and preferences, ensuring proper food safety and hygiene standards.
- Monitor medication reminders safely following care plans and HIPAA regulations, maintaining detailed documentation of medication administration.
- Support clients with cognitive impairments such as dementia or Alzheimer's disease through memory care strategies and specialized activities.
- Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe living environment to promote overall health and comfort.
- Document all care activities meticulously, including patient observations, progress notes, and adherence to individualized care plans.
Skills
- Proven experience working in group homes, assisted living facilities, nursing homes, hospice or home health environments.
- Strong patient observation skills with the ability to recognize subtle changes in condition or behavior.
- Knowledge of HIPAA regulations to ensure confidentiality and privacy of patient information.
- Competence in using medical equipment such as Hoyer lifts, blood pressure cuffs, thermometers, and other vital signs monitoring tools.
- Ability to perform CPR and first aid confidently in emergency situations; certification preferred.
- Excellent communication skills for working collaboratively with clients’ families, healthcare providers, and team members.
- Physical stamina for heavy lifting and manual handling tasks required during patient transfers or mobility assistance.
