Responsibilities
- Perform comprehensive patient assessments, including vital signs, physical examinations, and health history reviews to develop individualized care plans.
- Administer medications, injections, IV infusions, and other treatments following established protocols and safety standards.
- Manage wound care, catheterizations, tube feedings, and other specialized procedures with precision and attention to detail.
- Monitor patient progress and document clinical findings accurately using electronic health record (EHR) systems such as Epic or Athenahealth.
- Educate patients and caregivers on medication management, disease processes like dementia or Alzheimer's care, and lifestyle modifications to promote health and safety.
- Collaborate with physicians, case managers, and other healthcare professionals to coordinate ongoing care and discharge planning.
- Ensure compliance with HIPAA regulations and maintain strict confidentiality of all patient information.
Requirements
- Valid state licensure as a Licensed Practical Nurse (LPN).
- Proven experience working with individuals with disabilities or in senior care settings such as assisted living facilities or skilled nursing facilities.
- Critical care experience including ICU or emergency room background is highly valued.
- Familiarity with EMR/EHR systems like Epic, Cerner, or eClinicalWorks for accurate documentation.
- Knowledge of medical terminology, ICD coding (ICD-9/ICD-10), and long-term care regulations.
- Experience in IV infusion therapy, wound management, catheter care, and medication administration.
- Strong communication skills for effective caregiver interaction and client education.
- Ability to work independently in home environments while adhering to safety protocols and clinical confidentiality policies.
