Duties
- Deliver direct patient care by assisting with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support
- Monitor and observe patients’ conditions, including vital signs and overall health status
- Administer medications accurately following medical orders while maintaining strict adherence to HIPAA and home health care regulations
- Assist with medical procedures such as catheter care, wound dressing changes, and using patient transfer methods like Hoyer lifts or gait belts
- Maintain detailed medical documentation within Electronic Health Record (EHR) systems and ensure all records are current and compliant with privacy standards
- Support patients’ emotional well-being through compassionate communication and engagement in activities tailored to their needs
- Perform light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe environment for patients
Requirements
- Valid certification as an HHA, CNA, LPN, or RN with active licensure in good standing
- Proven experience working with geriatric patients or individuals with disabilities, dementia or Alzheimer’s care experience preferred
- Knowledge of medical terminology, medication administration protocols, and home healthcare regulations
- Ability to perform physical tasks such as heavy lifting and patient transfers safely and effectively
- Strong communication skills to coordinate with families and healthcare teams while maintaining patient confidentiality under HIPAA guidelines
- Certification in CPR and First Aid; Basic Life Support (BLS) certification is highly desirable
- Experience working with EMR/EHR systems for accurate documentation of patient care activities
- Flexibility to work in live-in employment settings or community-based environments as needed

Pay: $22.00 - $60.00 per hour
