Position Summary
We are seeking a compassionate, organized, and proactive Case Manager to join our treatment team. The Case Manager plays a vital role in supporting clients throughout their recovery by coordinating resources, advocating for their needs, and helping remove barriers to treatment success. This position works closely with clinicians, medical providers, and the management team to ensure clients receive comprehensive, individualized support both during treatment and as they transition back into the community.
Key Responsibilities
Client Care & Advocacy
- Provide ongoing case management services addressing clients' housing, transportation, employment, education, legal matters, insurance, benefits, and other psychosocial needs.
- Advocate for clients' needs within the treatment program and with community agencies.
- Assist clients with applications for community resources, benefits, financial assistance, and supportive services.
- Help clients complete forms, applications, phone calls, and other tasks that promote independence and life skills.
- Coordinate client appointments with outside providers and community resources as needed.
- Facilitate a rotating Life Skills Group.
Care Coordination
- Collaborate with clinicians, medical staff, and the management team to communicate client needs, progress, concerns, and barriers to treatment.
- Attend interdisciplinary staff meetings 2–3 times per week.
- Assist with PHQ-9 screenings as needed.
- Participate in treatment planning and care coordination to support positive client outcomes.
- Collaborate with primary clinicians to complete comprehensive discharge planning and discharge packets to promote long-term success following treatment.
Insurance Coordination
- Perform monthly verification of all client insurance coverage, including commercial insurance and Medicaid, for both Outpatient (OP) and Residential Treatment Center (RTC) programs.
- Identify and resolve insurance issues in a timely manner.
- Meet with clients experiencing insurance concerns to review coverage, facilitate calls with insurance companies, and assist in resolving barriers.
- Document all insurance-related interactions in the client's record.
- Maintain and update the Insurance Issues tracking spreadsheet with accurate and current information.
Documentation
- Complete all required documentation in a timely and accurate manner, including:
- Case management notes
- Discharge planning documentation
- Ensure documentation complies with organizational, regulatory, and payer requirements.
Qualifications
- Previous case management or behavioral health experience preferred.
- Knowledge of community resources, insurance benefits, and social service systems.
- Strong organizational and time management skills.
- Excellent written and verbal communication skills.
- Ability to work collaboratively within a multidisciplinary team.
- Strong problem-solving and advocacy skills.
- Ability to maintain confidentiality and professional boundaries.
