About the Role
You will provide comprehensive primary care to adult patients, including:
- Annual wellness visits and preventive care
- Management of acute illnesses
- Chronic disease management
- Medication management
- Patient education and lifestyle counseling
- Coordination of specialty referrals and follow-up care
- Accurate and timely documentation within our electronic medical record
Our providers practice collaboratively and have access to Internal Medicine physicians for consultation and mentorship while enjoying a high level of clinical independence.
Qualifications
- Current Illinois Nurse Practitioner or Physician Assistant license (or ability to obtain)
- DEA license (or ability to obtain)
- Excellent communication and interpersonal skills
- Passion for delivering patient-centered primary care
- New graduates are encouraged to apply—strong mentorship is available.
