Key Responsibilities:
- Provide comprehensive primary care to adult and Medicare Advantage patients
- Develop long-term patient relationships through a value-based care approach
- Manage chronic conditions, preventive care, and wellness initiatives
- Collaborate with a multidisciplinary care team focused on improving patient outcomes
Qualifications:
- MD or DO with an active Florida medical license (or eligibility)
- Board Certified in Internal Medicine or Family Medicine
- Value-Based Care experience preferred
- Geriatric Fellowship training welcomed
- Some clinics require or prefer Spanish Speaking
