Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using Electronic Health Record (EHR) systems such as Epic or Cerner.
- Administer medications, injections, IV infusions, and treatments following strict protocols to ensure safety and efficacy.
- Assist with complex procedures such as catheterization, wound care, tube feeding management, and ventilator support.
- Monitor patients’ conditions closely, recognize signs of acute pain or deterioration, and escalate care as needed through triage or collaboration with physicians.
- Document all patient interactions accurately in medical records while maintaining strict adherence to HIPAA and clinical confidentiality policies.
- Support discharge planning, patient education, and coordination of care for individuals with chronic conditions like dementia or Alzheimer's disease.
- Utilize critical thinking skills to evaluate diagnostic results, perform specimen collection/processing, and manage emergency situations in fast-paced environments such as Level I Trauma Centers or Emergency Rooms.
Qualifications
- Valid LPN license with current state licensure and certification in Basic Life Support (BLS).
- Proven experience working in diverse healthcare settings including hospital medicine, ICU, emergency medicine, long-term care facilities, or skilled nursing facilities.
- Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/ICD-10), and electronic health record systems like Epic or Athenahealth.
- Demonstrated ability to perform advanced nursing procedures such as IV insertion, phlebotomy, medication administration, and aseptic technique.
- Experience caring for geriatric patients with complex needs such as dementia or memory care residents is highly desirable.
- Excellent patient assessment skills with the ability to recognize urgent issues related to acute pain management or infectious disease control.
- Ability to adapt quickly to new clinical environments while maintaining high standards of patient care and safety.
