Integrated Care Nurse PractitionerLocation: Tacoma / Greater Seattle Area, WashingtonSalary: $120,000–$145,000 annuallyWork Setting: Field-Based | In-Home CareJob OverviewThe Integrated Care Nurse Practitioner provides personalized, value-based medical care to patients living primarily with chronic kidney disease and end-stage renal disease. This field-based role includes in-home patient visits, comprehensive health assessments, care coordination, preventive management, and ongoing treatment planning.The Nurse Practitioner will work closely with physicians, field nurses, community health workers, primary care providers, specialists, and ancillary services to improve patient outcomes, close gaps in care, reduce avoidable hospitalizations, and enhance patients’ quality of life.This is not an office-based position. Candidates must be comfortable traveling throughout their assigned territory and should live in or near the Greater Seattle and Tacoma area.Preferred candidate locations include: TacomaNewcastleSeattle and South SeattleShorelineBellevueMedinaIssaquahRedmondTravel RequirementsCandidates must be comfortable traveling throughout the assigned territory for in-home patient visits.The position may also require quarterly travel for up to three days to locations approximately 200 miles away. Approved hotel accommodations and rental-car expenses may be reimbursed in accordance with company policy.
