Abby Dot Homecare is seeking a highly skilled and clinically competent Registered Nurse (RN) to join our home health team on a part‑time/PRN basis. The ideal candidate is a professional who demonstrates strong clinical judgment, exceptional assessment skills, and the ability to deliver high‑quality care in a home‑based setting.
This role focuses on Start of Care (SOC) admissions, Recertification assessments, Resumption of Care (ROC), Supervisory Visits and Advance Care Visits, ensuring continuity, safety, and excellence in patient outcomes.
Essential Duties & Responsibilities
- Start of Care (SOC) Admissions — complete comprehensive OASIS assessments, establish individualized Plans of Care, and initiate services in accordance with CMS and agency standards
- Recertification Visits — evaluate patient progress, update care plans, and ensure ongoing eligibility for home health services
- Resumption of Care (ROC) — reassess patients following hospitalization or significant change in condition
- Advance Care Visits — provide skilled follow‑up, clinical oversight, and patient/caregiver education
- Perform skilled nursing interventions including wound care, medication management, disease‑specific teaching, and safety assessments
- Maintain accurate, timely, and compliant documentation within agency guidelines
- Communicate effectively with the clinical manager, interdisciplinary team, and patient caregivers to support coordinated care
- Uphold all regulatory, clinical, and ethical standards of home health practice
Qualifications
- Active Texas Registered Nurse (RN) license in good standing
- Minimum 1 year of home health experience preferred (OASIS experience strongly valued)
- Strong clinical assessment, critical thinking, and documentation skills with kinnser
- Ability to work independently in the field with professionalism and reliability
- Valid driver’s license and dependable transportation
- Familiarity with Houston communities and surrounding areas is an advantage
