As an LPN, you’ll provide comprehensive nursing care while overseeing clinical operations on your unit. You’ll collaborate with an interdisciplinary team to deliver safe, effective, and resident-centered care.

Perform assessments and develop individualized care plans

Administer medications and treatments per physician orders

Monitor resident conditions and document changes accurately

Collaborate with physicians, therapists, and care teams

Educate residents and families on care plans

Supervise and support CNAs
-
Ensure compliance with clinical standards and infection control

Active Licensed Practical Nurse (LPN) license in good standing

Long-term care or rehabilitation experience preferred

Strong leadership, communication, and documentation skills

Compassionate, resident-first mindset

Current BLS/CPR certification required (ACLS a plus)
