Job Summary
Wellness Care is hiring a dedicated RPM Specialist to own the day-to-day execution, tracking, and performance of our Remote Patient Monitoring program.
This role is responsible for making sure eligible patients receive the right device, the provider’s signed RPM order is documented before shipment, devices are provisioned and tested, updated care plans are shipped with devices, patient readings are tracked, alerts are routed appropriately, and RPM time is documented accurately.
This is a high-accountability healthcare operations role for someone who is organized, detail-oriented, comfortable with technology, and able to manage cross-functional workflows across Care Pros, providers, device provisioning, shipping, billing, and clinical escalation.
About Wellness Care
Wellness Care is building a new model of chronic care management for Medicare patients. We support patients with whole-person care, condition-specific care teams, ongoing care management, and remote monitoring tools that help our care teams better support patients between calls and visits.
Key Responsibilities
RPM Program Operations
- Own the daily RPM patient pipeline from device request through activation and ongoing monitoring.
- Maintain accurate trackers for RPM candidates, patient consent, provider orders, device shipment, device activation, readings, alerts, and RPM time.
- Ensure all RPM requests are complete, compliant, and ready for provider review.
- Coordinate across Care Pros, providers, RPM IT/device provisioning, care plan shipping, billing, and clinical escalation teams.
- Run daily or weekly RPM check-ins to keep the program moving and remove bottlenecks.
Patient Eligibility and Documentation
- Confirm patients meet Wellness Care RPM readiness requirements before shipment.
- Verify patient consent is documented before an RPM device is sent.
- Confirm shipping address, insurance information, and validated Medicare Beneficiary Identifier (MBI) are complete.
- Ensure no RPM device is shipped without a signed provider order.
- Audit RPM documentation for required elements, including condition being monitored, device ordered, medical necessity, reading frequency, patient consent, and care team monitoring plan.
Device Provisioning and Shipment Coordination
- Coordinate with the RPM IT/device resource to assign devices to the correct patient.
- Confirm device ID, IMEI, or other device identifiers are entered correctly.
- Confirm each device is tested and transmitting before shipment.
- Coordinate with the care plan shipping resource to ensure the updated care plan is printed and shipped with the device.
- Track device shipment status, delivery, first reading completion, device issues, and replacement needs.
Billing Readiness and RPM Time Tracking
- Partner with billing to support accurate RPM revenue capture.
- Prepare weekly RPM reporting for leadership.
Training and Patient Experience
- Train Care Pros on how to explain RPM to patients in simple, patient-friendly language.
- Reinforce that devices transmit through built-in cellular service and do not require Wi-Fi, a smartphone, an app, or Bluetooth setup.
- Reinforce that there is no cost to the patient for the RPM device or device cellular service.
- Create or improve scripts, FAQs, troubleshooting guides, and patient education materials.
- Identify recurring patient confusion and improve the workflow or training materials.
Required Qualifications
- 2+ years of experience in healthcare operations, care coordination, patient monitoring, chronic care management, population health, medical office operations, or similar healthcare support role.
- Strong comfort working in EMR/EHR systems and patient tracking tools.
- Excellent attention to detail and documentation discipline.
- Ability to manage queues, trackers, handoffs, follow-ups, and daily operational workflows.
- Strong written and verbal communication skills.
- Ability to work cross-functionally with providers, care teams, billing, IT, and operations.
- Comfortable training team members and improving workflows.
- Patient-centered mindset with strong follow-through.
Preferred Qualifications
- Experience with Remote Patient Monitoring, Chronic Care Management, Medicare care management, home health technology, device logistics, or virtual care operations.
- Experience with Medicare documentation, billing readiness, or compliance-oriented workflows.
- Experience building SOPs, dashboards, trackers, or audit processes.
- Medical assistant, LPN/LVN, care coordinator, patient navigator, or clinical support background preferred but not required.
- Experience in a fast-growing healthcare company or startup environment.
Success Metrics
- 100% of shipped RPM devices have signed provider order before shipment.
- 100% of shipped RPM devices have documented patient consent.
- 100% of shipped RPM devices have confirmed shipping address.
- 100% of shipped RPM devices are provisioned and test-transmission confirmed before shipment.
- 100% of shipped RPM devices include the updated care plan and patient insert card.
- 90%+ of shipped devices complete first reading within 7 days of delivery.
- 80%+ of active RPM patients achieve at least 2 reading days per month.
- 50%+ of active RPM patients achieve 16+ reading days per month after stabilization.
- 95%+ RPM time entries are accurately categorized and not double-counted with CCM.
- Weekly RPM dashboard delivered accurately and on time.

Pay: $65,000.00 per year
