Job Overview
We are seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) to join our healthcare team. The ideal candidate will have a strong background in patient care, particularly in residential care, As an LVN, you will be responsible for providing high-quality nursing care to patients, assisting with their daily needs, and ensuring adherence to established healthcare standards.
Duties
- Facilitate an integrated, cohesive and supportive treatment approach and attitude. Act as a role model and participate as a member of a multi-disciplinary treatment team with other staff.
2. Provide daily supports and services as detailed in the client’s health care plan. Some examples include: diabetes monitoring, medication administration per the scope of LPT’s practice, and wound care.
3. Chart and report any significant changes in condition and general progress of consumers to Program Administrator, next shift clinical staff, Medical Director, physicians, and psychiatrists.
4. Follow and adhere to all medication Policies and Procedures regarding dispensation and administration, record keeping, storage and accountability for inventory.
5. Be familiar with each resident's IPP goals, IBSP objectives and methods for achieving these goals and objectives. Safely and effectively carry out Individual Emergency Intervention Plans (IEIP) as designed.
6. Assist clients in their daily activity schedules in accordance with individual treatment plans.
7. Assist clients in activities of daily living, including hygiene, nutrition, community integration, and communication.
8. Assist in the admission process and preparation of all required forms on clients accepted into the program, including initial intakes and treatment plans, in an accurate and timely manner.
9. Complete accurate and timely charting in accordance with training on DDS/CCL and agency guidelines and requirements.
10. Participate in daily individual support, medication support, education programs, and case management services for clients.
11. Develop cross-cultural awareness of culturally diverse population and apply appropriate interventions in assisting staff with socio/cultural issues.
12. Provide supervision of individuals, have a working knowledge of, and provide crisis prevention and intervention for residents.
13. Protect client rights and maintain consumers' confidentiality. Prioritize consumers’ safety at all times.
14.Work the hours required and as scheduled by the Director of Operations or Program Administrator, including overnights, weekends, holidays, occasional fill-in for other staff, and on-call responsibilities. This includes mandatory trainings that may fall outside of the usual scheduled shift.
15. Contribute to the ongoing maintenance, safety, and housekeeping of the facility.
16. Participate in in-service training provided by the Regional Center and ESSENTIAL BEHAVIOR SUPPORTS, INC.
17. Complete twenty required continuing education, within each twelve month period following the assumption of duties. Complete all additional topics and hours of training required by agency policy, Title 17 or Title 22 regulations.
18. Provide for the care and safety of individuals without physical or verbal abuse, exploitation or prejudice. Report any incidents or suspected incidents of abuse according to current law and ESSENTIAL BEHAVIORAL SUPPORT, INC. Policies and Procedures.
19. Report observations or evidence of violations of any personal rights specified in Titles 17 and 22, and any unusual or special incidents as defined by Title 17 and 22 per training by SARC.
20. Provide community linkage and community public relations when necessary.
21. Perform all other duties and responsibilities as assigned by supervisor.
22. Must obey all federal, state, and local laws and ESSENTIAL BEHAVIORAL SUPPORT, INC. Policies and Procedures.
23. Comply with the EBSI policy requirement to submit proof of valid CDL, maintain good driving record (as evidenced by initial submission of DMV report and ongoing approval as agency driver), and submit proof of auto insurance for current period of coverage.
24. Must stay until relief arrives, and stay on shift in the event of the emergency.25. This position requires flexibility and the staff assigned to work the shift may be changed to another location with OR without notice, in order to meet the supervision needs of the individuals served in ANY location. We are not able to commit to dedicated locations due to the nature of this work.
Requirements
- Valid Licensed Vocational Nurse (LVN) license in the state of practice.
- Knowledge of anatomy and physiology relevant to patient care.
- Experience in occupational health, home care, or senior care is preferred.
- Proficiency in sonography techniques is a plus.
- Strong communication skills and the ability to work effectively within a team.
- Attention to detail and strong organizational skills.
- Ability to manage multiple tasks while maintaining a high standard of patient care.
