The Home Health Patient Intake Specialist is responsible for managing new patient referrals from start to finish. This role serves as a key connection between referral sources, patients, clinicians, and insurance providers. The Intake Specialist ensures referrals are processed accurately, timely, and in compliance with payer and regulatory requirements.
Previous home health experience is not required. We are willing to train the right candidate who is organized, reliable, and comfortable working in a fast-paced healthcare environment.
Key Responsibilities
- Receive and process new patient referrals from hospitals, physician offices, SNFs, and other referral sources
- Review referral information for completeness and accuracy
- Obtain and verify patient demographics, insurance information, and required documentation
- Perform insurance verification and eligibility checks
- Communicate with referral sources to obtain missing information or clarify orders
- Coordinate with clinical staff to schedule Start of Care (SOC) visits
- Enter referral and patient information into the EMR accurately and timely
- Track referral status and follow up to ensure timely admission
- Communicate with patients and families regarding admission process and next steps
- Maintain compliance with HIPAA and agency policies
- Assist with other intake-related administrative tasks as needed
Qualifications
- High school diploma or equivalent required
- No prior home health or healthcare intake experience required
- Strong organizational and time-management skills
- Attention to detail and accuracy
- Comfortable speaking with patients, families, and medical offices by phone
- Basic computer skills and ability to learn EMR systems
- Ability to prioritize tasks and meet deadlines
- Professional, courteous, and dependable
Preferred (But Not Required)
- Experience in healthcare, medical office, or customer service role
- Familiarity with insurance verification or medical terminology
- Bilingual (English/Spanish)
