Job Summary
Join a young, mission-driven primary care organization in Washington, DC as a Lead Family Nurse Practitioner — earning $125,000–$145,000 base salary, working a hybrid Monday–Friday schedule with three days on-site and two days telehealth from home, and practicing with full practice authority from day one under DC's full scope of practice licensure. No collaborative agreement, no supervision requirement, no waiting period.
This is a founding team clinical leadership role inside four small clinics built into converted apartments in DC public housing — serving families who have historically had almost no reliable access to a provider who knows them. You'll carry your own panel, serve as the senior clinical voice, and help shape the care model as the organization grows.
Responsibilities
Direct Patient Care
- Deliver comprehensive primary care across the lifespan including well child visits, immunizations, physicals, screenings, and chronic disease management
- Diagnose, treat, and manage acute and chronic conditions with emphasis on prevention and complex multi-condition patients
- Prescribe and manage medications including controlled substances
- Coordinate care with specialists, hospitals, and community organizations
- Deliver care through both on-site visits and telehealth
Clinical Leadership
- Serve as the senior advanced practice provider and clinical resource for the team
- Provide clinical coverage and decision-making authority when executive leadership is unavailable
- Develop clinical workflows, care pathways, and operational processes as the model matures
- Mentor and support NPs, nurses, MAs, and care coordinators
- Contribute to recruitment, onboarding, and training of new clinical staff
Population Health & Quality
- Lead chronic disease management and care gap closure initiatives across the patient panel
- Support quality improvement, documentation quality, and performance measurement
- Help address social determinants of health through partnership with housing and community organizations
- Contribute to value-based care and population health work
- Participate occasionally in community outreach events at your clinic site
Requirements
- Active, unrestricted Washington, DC NP license required (or ability to obtain before start)
- FNP certification through ANCC or AANP required — FNP required due to pediatric patients on the panel, no AGNP
- Active DEA registration required
- MSN, DNP, or equivalent advanced nursing degree required
- Minimum 5 years of clinical NP experience required
- Demonstrated chronic disease management experience required
- Experience serving Medicaid or underserved populations required
- Comfortable seeing pediatric patients including well child care, vaccines, and physicals
- Proficiency with EHR systems required
Compensation & Benefits
- $125,000–$145,000 annual base salary (commensurate with experience)
