DJob description:
RESPONSIBILITIES: Assess patient health problems and needs, develop and implement nursing care plans, and maintain medical record. Administer nursing care during preventive care services to ill, injured, convalescent, or disabled patients. Advise patients on health maintenance and disease prevention, and provide medical case management. Maintain accurate, detailed reports and records. Participate and oversee performance and quality improvement activities. Work collaboratively with LPNs Medical Assistants in support of clinical providers and patients’ needs.

QUALIFICATIONS:
Current New York State Registered Nursing License (BS preferred)
CPR Certification
Infection Control Certification
Bilingual Spanish Preferred
Excellent written, verbal and interpersonal skills
