Axzons is seeking a skilled and proactive Field Registered Nurse (RN) to provide in-home patient assessments, care planning, and clinical oversight.
This role is critical to:
- Ensuring high-quality patient care
- Supporting MLTC case activation
- Enabling fast and compliant patient onboarding
The ideal candidate is organized, responsive, and experienced in home care / MLTC environments in New York.
Key Responsibilities 1. Patient Assessments & Admissions
- Conduct initial in-home assessments for new patients
- Evaluate:
- Medical condition
- Functional status
- Care needs
- Develop and implement plan of care (POC)
This is critical for starting cases quickly
2. Care Planning & Clinical Oversight
- Create and update individualized care plans
- Supervise Home Health Aides (HHAs) / caregivers
- Ensure services align with:
- MLTC authorizations
- Patient needs
- Conduct periodic reassessments
3. Coordination with MLTC Plans & Care Teams
- Communicate with:
- MLTC care managers (Centers Plan, VNS, Fidelis, etc.)
- Social workers
- Physicians
- Ensure timely documentation for authorizations
- Support approvals and case continuity
4. Rapid Case Activation (High Priority)
- Conduct admissions within 24–48 hours of referral
- Work closely with scheduler to:
- Start cases quickly
- Avoid delays
5. Field Supervision of Caregivers
- Perform supervisory visits of HHAs/PCA staff
- Ensure:
- Proper care delivery
- Compliance with plan of care
- Provide training or corrective feedback as needed
✅ 6. Compliance & Documentation
- Maintain accurate and timely documentation
- Ensure compliance with:
- NY State Department of Health (DOH)
- Agency policies
- MLTC requirements
- Prepare documentation for audits
7. Incident Reporting & Quality Assurance
- Identify and report patient incidents or changes in condition
- Escalate urgent clinical issues
- Participate in quality improvement initiatives
QualificationsRequired:
- Active New York RN License
- Minimum 1–2 years clinical experience
- Experience in:
- Home care OR
- MLTC OR
- Hospital discharge / community care
- Strong assessment and documentation skills
- Ability to work independently in the field
- Valid driver’s license (or reliable transportation)
Preferred:
- Experience with MLTC plans in NY
- Knowledge of:
- DOH compliance standards
- Bilingual (Spanish, Russian, Haitian Creole, Hindi, etc.)
- Prior supervisory experience
