About the Role
Home Care For You, Inc. is seeking a compassionate and experienced Field Nurse Supervisor (Registered Nurse) to join our home health team. In this role, you will oversee patient care in the field, conduct clinical assessments, coordinate care plans, and supervise Home Health Aides (HHAs) to ensure high-quality, compliant home care services.
This position plays a key role in ensuring patients receive safe, effective, and well-coordinated care in accordance with physician orders, agency policies, and New Jersey state regulations.
Key Responsibilities
- Conduct initial home care patient visits and ongoing reassessments
- Initiate, implement, and update individualized plans of care under physician orders
- Perform comprehensive patient assessments and document findings accurately
- Monitor patient symptoms, reactions, and overall progress
- Educate patients and caregivers on conditions, medications, and care plans
- Teach in-home care techniques to patients and family members
- Coordinate patient services across disciplines and care teams
- Supervise and provide onsite oversight of Home Health Aides (HHAs)
- Notify physicians, DON, PT, and case management of any changes in patient condition
- Ensure all clinical documentation is completed accurately and timely
- Participate in case conferences with interdisciplinary teams
- Discharge patients when clinical goals and criteria are met
- Maintain compliance with agency policies and New Jersey state regulations
- Participate in continuing education, in-services, and professional development
- Be available for on-call support and home visits as needed
Qualifications
- Active Registered Nurse (RN) license in good standing in New Jersey
- Minimum of 1 year RN experience in the United States (home health experience preferred)
- Must successfully pass Home Care examination
- Must pass High-Tech exam when applicable
- Validation of high-tech skills (e.g., IV therapy, ventilator care) when applicable
- Strong communication and interpersonal skills with patients, families, and staff
- Commitment to ongoing education per agency and NJ Board of Nursing requirements
- Ability to participate in on-call rotation and field-based visits
Physical & Work Requirements
This role requires the ability to:
- Communicate effectively (speech, hearing, and comprehension)
- Review and document clinical information in electronic and paper formats
- Operate standard office and clinical equipment (phone, computer, etc.)
- Perform physical tasks necessary for field-based nursing duties
- Travel frequently to patient homes in varying conditions
Work environments may include:
- Office settings with standard conditions
- Patient homes that vary in cleanliness, temperature, and accessibility
- Occasional interaction with distressed or high-need patients and families
