Job Duties:
· Act as a liaison between DES and the community to identify community resources and develop volunteer -based activities in consideration of the needs, abilities, and interests of the participant.
· Coordinate and complete participant assessments
· Coordinate, develop and review Individual Support Plans (ISP’s) and ensure the implementation of individual program plans and program activities
· Develop planned program activities.
· Participate in interdisciplinary team meetings
· Supervise and maintain a leadership role with facility staff and volunteers
· Participate in staff annual performance reviews
· Be knowledgeable of the duties and responsibilities of DSP I and DSP II and be able to assume those duties, if necessary.
· Be aware of all DES and Office of Developmental Programs regulations, so that the appropriate response is made in a given situation.
· Utilize DES time appropriately to ensure quality service to the participants.
· Maintain current Medication Administration Certification and dispense medication to participants.
· Maintain Medication Administration Trainer Certification
· Maintain Certified Investigator Certification
· Understand and respect the confidentiality of the participant, the participant’s family and other staff at all times
Requirements:
- A minimum of an associate's degree or 60 credit hours from an accredited college or university and 4 years of work experience working directly with persons with disabilities. or A bachelor's degree from an accredited college or university and 2 years of work experience working directly with persons with disabilities. or A master's degree or above from an accredited college or university and 1 year of work experience working directly with persons with disabilities.
- Must be 18 years or older and valid driver's license is required.
