TELAMON|TRC COMMITMENT TO TRAUMA INFORMED CARE:
Telamon|TRC has taken steps to shift our organization into a Trauma-Informed Care (TIC) model. This changes our perceptions about the individuals that we work with, employees and customers, including families and children. A trauma-informed system of care provides services that empower employees and customers, including families and children, to feel safe, welcomed and engaged at all levels of organizational interaction. Exchanges that could be potentially retraumatizing, or trigger traumatic reactions require thoughtfulness and non-judgement. Ongoing, as an organization, we collaborate to educate employees about the effects of violence, victimization and trauma on individuals. This model supports a culture of safety for employees and customers, including families and children.
POSITION SUMMARY:
Ensures customers are directed to the most appropriate community resources to meet their needs. Provides information to community partners to support the ongoing distribution of resources based on local needs.
DUTIES AND RESPONSIBILITIES:
Receives referrals and determines eligibility and enrolls customers into appropriate programs with the intake process.
Assesses customer needs through comprehensive assessments.
Makes referrals and encourages customer to take advantage of relevant programs, in order to enhance their self-sufficiency and meet their immediate needs.
Coordinates services through case management. Follows up with customers to determine use of
resources and/or need for referrals to additional resources.
Maintains an accurate, current, and comprehensive regional resource data base of health, human
service and other community-based organizations’ service information.
Disseminates updated and relevant marketing about Regional Resource Collaborative services to community-based organizations that may provide financial and/or other types of assistance of potential interest to program customers.
Initiates and develops strategies to enhance inter-agency collaboration to better serve customers.
Maintain individual customer files.
Analyze data and develop reports for dissemination.
Participate in task force meetings and other related meetings or trainings.
Actively engages in staff meetings and conference calls.
Participate in professional development activities.
Other duties as may be assigned by supervisor.
SAFETY DUTIES AND RESPONSIBILITIES:
Ensuring a secure workplace is a collective effort. All employees are responsible for adhering to established safe work practices and completing all mandatory safety training by the assigned due date.
SUPERVISORY RESPONSIBILITIES:
This position has no direct supervisory duties but is expected to train or explain work procedures to others.
ROLE QUALIFICATIONS:
Education and Experience Requirements:
Minimum of an Associate’s Degree with three years of experience.
Bachelor’s degree with one year experience preferred.
Extensive knowledge of community resources and services.
Demonstrated ability to work with volunteers and community organizations.
Experience providing presentations to groups.
Must have own transportation, a valid driver’s license, and vehicle insurance.
Microsoft Office products and on-line client management systems.
Bilingual ability preferred.
Problem-solving, arithmetic calculations
Ability to effectively communicate in a professional manner, both orally and in writing
Ability to de-escalate and handle high stress situations.
