Job Summary:
We are seeking a Board-Certified or Board-Eligible Neurology Physician (MD/DO) to provide on-site neurology consultations for residents in skilled nursing and long-term care settings. The physician will evaluate and manage complex neurologic disorders while collaborating with Advanced Practice Providers and facility teams.
Responsibilities:
- Provide on-site neurology consultations in SNF/LTC settings
- Diagnose and manage neurologic conditions including stroke, epilepsy, Parkinson’s disease, dementia, neuropathy, and movement disorders
- Perform comprehensive neurologic examinations and cognitive assessments
- Order, interpret, and act upon diagnostic studies including imaging and laboratory tests
- Manage neurologic medications and treatment plans
- Supervise and collaborate with Nurse Practitioners and interdisciplinary teams
- Participate in care planning, quality improvement, and hospital readmission reduction initiatives
- Ensure accurate and regulatory-compliant documentation in EMR systems
Qualifications:
- MD or DO degree from an accredited medical school
- Completion of an accredited Neurology residency
- Active and unrestricted New York State medical license
- DEA registration
- Board Eligible or Board Certified in Neurology
- Experience in SNF/LTC or post-acute care preferred
