Job Summary
Join our dedicated healthcare team as a Home Health Aide (HHA), where you will play a vital role in providing compassionate, personalized care to individuals in their homes. Responsibilities
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts when necessary.
- Monitor patient health by taking vital signs such as blood pressure, pulse, temperature, and respiration; document medical records accurately using Electronic Health Record (EHR) systems while maintaining HIPAA compliance.
- Support medication administration under supervision, ensuring proper timing and dosage as prescribed by healthcare providers.
- Provide specialized care for patients with dementia or Alzheimer's disease, including memory care techniques and behavioral management strategies.
- Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe living environment for the patient.
- Observe and report any changes in patient condition promptly to healthcare professionals; maintain detailed medical documentation for ongoing care plans.
- Use medical terminology appropriately when recording notes and communicating with other caregivers or medical staff.
- Assist with medical procedures such as catheter care or hospice support when required; operate medical equipment safely and effectively.
- Support patients during community outings or appointments if needed; work collaboratively within a team to ensure comprehensive care delivery.
Experience
- Prior experience in home health care or assisted living environments is highly preferred.
- Demonstrated ability to work with individuals with developmental disabilities or chronic conditions such as dementia or Alzheimer's disease.
- Experience working in nursing homes or long-term care facilities is advantageous.
- Familiarity with EMR (Electronic Medical Records) or EHR systems for documentation purposes.
- Knowledge of CPR certification, and basic first aid skills are required.
- Proven ability to handle sensitive situations with compassion while maintaining patient dignity and confidentiality under HIPAA regulations.
