Social Services Coordinator (Part-Time)
LivActive Adult Day Health is seeking a compassionate, organized, and motivated Social Services Coordinator to join our growing team. This position plays an integral role in supporting our participants and their families by providing psychosocial assessments, care planning, counseling, and connecting individuals with community resources.
Schedule
- Part-Time
- Monday-Friday
- 10:00 AM-2:00 PM (20 hours per week)
- This position is expected to transition to full-time as our participant census grows.
Compensation
- Competitive hourly rate (based on experience)
Responsibilities
- Complete psychosocial assessments and ongoing participant evaluations.
- Develop and update individualized care plans as part of the interdisciplinary team.
- Provide supportive counseling to participants and families.
- Coordinate referrals to community resources and supportive services.
- Assist participants and caregivers with adjustment, advocacy, and discharge planning as needed.
- Maintain timely and accurate documentation in accordance with Massachusetts Department of Public Health and MassHealth regulations.
- Collaborate closely with nursing, activities, rehabilitation providers, physicians, caregivers, and community agencies.
Qualifications
- Massachusetts Licensed Social Worker (LSW), Licensed Certified Social Worker (LCSW), or Licensed Independent Clinical Social Worker (LICSW) required.
- Bachelor's or Master's degree in Social Work from an accredited college or university.
- Experience working with older adults, adults with disabilities, healthcare, adult day health, geriatrics, case management, or long-term care preferred.
- Excellent communication, organizational, and documentation skills.
- Ability to work collaboratively in a multidisciplinary team.

Responsibilities
- Conduct thorough patient assessments to identify social needs, including mental health, housing, financial assistance, and family support.
- Coordinate with community resources such as housing agencies, mental health providers, and social service organizations to facilitate patient access to necessary services.
- Provide counseling support for individuals and families dealing with complex issues such as addiction, grief, or chronic illness through clinical counseling techniques and motivational interviewing.
- Assist in discharge planning by ensuring patients have appropriate post-care arrangements and understand available community resources.
- Maintain accurate medical documentation and social work records in compliance with HIPAA regulations and clinical ethical guidelines.
- Collaborate with interdisciplinary teams to develop personalized care plans that incorporate behavioral health strategies like cognitive behavioral therapy or applied behavior analysis when appropriate.
- Lead crisis intervention efforts for patients experiencing acute emotional distress or behavioral health emergencies, ensuring safety and timely intervention.
Skills.
- Experience working with diverse populations including geriatric patients, individuals with developmental disabilities or autism, and those requiring Alzheimer’s or dementia care.
- Skilled in crisis management, discharge planning and case management.
- Experience working with individuals from diverse cultural backgrounds to ensure equitable access to services.
