SERVICE DESCRIPTION
Deliver direct care and support to help our participants resolve and manage issues in their lives. Ensure COMAR compliance and supervision of all licensed clinical staff.
SPECIFIC DUTIES/RESPONSIBILITIES:
1. Assist with coordination of intakes, diagnostic evaluations, transfers, and discharge meetings;
2. Writing contact notes per visit with member, Individual Treatment Plan (ITP), progress notes and discharge summaries;
3. Coordinate Individual Treatment Plan (ITP) meetings as deemed necessary by Program Director;
4. Collaborate with other Treatment Team members (PRP, Medical Director, and Program Director);
5. Contact appropriate agencies and other parties in suspected cases of abuse and neglect and develop written reports;
6. Conduct individual, group and family therapy/counseling as needed;
7. Maintain contact with caregivers, case managers, attorneys, etc;
8. Facilitate therapeutic groups with clients to assist in the development of social and interpersonal skills;
9. Ensure overall health, safety and well being of participants;
10. Support participants and their families by informing them of available community services;
11. Refer participants, as necessary, to agencies providing community services;
12. Assist in the delivery of other required program services, including meeting the psychosocial needs of participants;
13. Provide weekly clinical supervision to graduate level practitioners.
14. Maintain documentation of supervision sessions.
15. Provide clinical insight and professional development to supervisees.
16. Complete supervisor portion of application for licensure exams.
17. Participate in monthly Clinical team meeting and provide clinical insight and treatment strategies.
18. In consultation with the medical director and program staff, responsible for identifying staff training needs required to maintain program standards;
ASSESSMENT AND EVALUATION:
1. Reviews client’s ITP/ITRP to determine goal and objectives that need to be carried out in the OMHC;
2. Observe staff and client interaction.
TRAINING:
1. Conduct in-service training for staff as directed by CEO
2. Participate in any Appereon Health Services, LLC recommended training
QUALIFICATIONS:
1. Valid and current license in the state of Maryland to include: LCSW-C, LPC, LCPC, LMFT, LCADC and Licensed Psychologist within the State of Maryland;
2. Have at least three years of experience or training working with clients with pediatric, adults, and elderly with or without developmental disabilities whose condition, due to mental and physical health needs and characteristics, require intensive services;
3. Provide individual and group supervision contact hours for graduate level practitioners.
4. Be at least 21 years of age;
5. Be able to read and write the English language;
6. Must be a US citizen or alien who is lawfully authorized to work in the US; and
7. Satisfactory physical examination by a licensed practitioner including, but not limited to freedom from communicable disease as confirmed by an annual PPD skin test/chest x-ray or documentation from a physician stating that the person is free from communicable disease.
8. Demonstrated knowledge and /or experience with:
a. Co-occurring disorders treatment
b. Integrated Behavioral Health
c. Prevention and intervention services
9. Excellent verbal and written communication skills;
10. Ability to serve as role model and provide leadership necessary to ensure a positive team approach;
