Position Overview
We are seeking a compassionate, dedicated, and resourceful Case Manager to join our Interim Housing team. In this role, you will provide comprehensive case management services to individuals experiencing homelessness and serious mental illness, helping them achieve stability, access critical resources, and work toward long-term housing and wellness goals.
The ideal candidate is skilled in building trusting relationships, navigating complex systems of care, and advocating for vulnerable populations. This position requires a strong commitment to trauma-informed, client-centered services and the ability to collaborate effectively within a multidisciplinary team.
Key Responsibilities
- Conduct comprehensive client assessments to identify needs, strengths, barriers, and goals.
- Develop and implement individualized service plans that support housing stability, recovery, and self-sufficiency.
- Provide ongoing case management, crisis intervention, and behavioral health support utilizing a Trauma-Informed Care approach.
- Connect clients with community resources, including healthcare, mental health services, substance use treatment, benefits, employment services, and housing programs.
- Utilize motivational interviewing techniques to support client engagement, personal growth, and goal achievement.
- Coordinate care with internal staff, community providers, and partner agencies to ensure a holistic and integrated service approach.
- Maintain accurate and timely documentation, including case notes, assessments, service plans, and progress reports, in accordance with agency policies and regulatory requirements.
- Participate in team meetings, case conferences, and program reviews to support quality service delivery.
- Advocate for clients' rights, needs, and access to services within the community and relevant systems of care.
- Monitor client progress and adjust service plans as needed to support positive outcomes.
Qualifications
- Bachelor's degree in Social Work, Psychology, Human Services, or a related field preferred; equivalent combination of education and relevant experience may be considered.
- Experience providing case management, crisis intervention, behavioral health services, or social services strongly preferred.
- Experience working with individuals experiencing homelessness and/or housing instability is required.
- Knowledge of serious mental illness, substance use disorders, and dual-diagnosis populations preferred.
- Familiarity with addiction recovery principles, mental health services, and community-based resources.
- Strong interpersonal, communication, and advocacy skills with the ability to engage individuals from diverse backgrounds.
- Ability to work independently while collaborating effectively within a multidisciplinary team.
- Excellent organizational skills and attention to detail, including the ability to maintain accurate and confidential records.
- Experience with Electronic Health Records (EHR) and Homeless Management Information Systems (HMIS) preferred.
- Demonstrated ability to build rapport, maintain professional boundaries, and support clients through challenging circumstances.
