Full-Time RN Case Manager – Hybrid Schedule (Friday–Tuesday)

About the Role
CritiCare Home Health & Nursing Services is seeking a dedicated and highly organized Registered Nurse Case Manager to oversee the skilled side of our home health operations while also supporting our school-based nursing programs. This is a full-time hybrid position—working between the office, from home, and in the field.
Weekend availability is required (Saturday and Sundy)

Key Responsibilities but not limited to:
- Manage and coordinate skilled home health cases, ensuring high-quality, compliant patient care.
- Conduct field visits as needed for assessments, follow-ups, and care plan oversight.
- Collaborate closely with families, caregivers, physicians, and interdisciplinary team members.
- Support CritiCare’s school nursing programs by assisting with staffing, communication, and care coordination.
- Maintain accurate documentation and ensure timely submission of all required clinical records.
- Serve as a resource for nurses in the field, offering guidance, problem-solving, and clinical support.

Qualifications
- Active RN license in the state of Pennsylvania (or ability to obtain).
- Previous experience in home health, case management, pediatrics, or school nursing preferred.
- Strong clinical judgment, communication skills, and ability to work independently.
- Comfortable managing a hybrid schedule (office, home, and field).
- Reliable transportation for field visits.
- Ability to work Friday–Tuesday, including weekends.

Pay: $70,000.00 - $85,000.00 per year
