What You’ll Do
As a Palliative Care Physician, you’ll play a pivotal role in delivering patient-centered care in LTAC and acute care facilities. Your responsibilities will include e:
- Leading daily rounds with the support of an NP, who assists with patient visits, documentation, and follow-ups.
- Managing complex symptoms of serious illnesses, prescribing medications, and monitoring treatment effectiveness.
- Collaborating with specialists (e.g., oncologists, cardiologists) and primary care providers to integrate palliative care into comprehensive treatment plans.
- Guiding patients and families through complex medical decisions, facilitating open communication about care goals and advance care planning.
- Developing proactive, individualized care plans to ensure patient safety and optimal outcomes.
- Documenting care seamlessly in our EHR system and supporting smooth transitions, including hospital-to-home care.
What We’re Looking For
We’re seeking a physician who brings both expertise and empathy to their practice. Our ideal candidate has:
- A Doctor of Medicine (M.D.) from an accredited institution.
- Board Certification or Eligibility in Palliative Care.
- An active California Medical License and active DEA License.
- Experience in acute care or LTAC settings (preferred but not required).
- Exceptional communication skills to connect with patients, families, and interdisciplinary teams.
- A compassionate, patient-first approach to care delivery.
