We are looking for self-motivated individuals who are excited to work for an innovative healthcare company to improve patient outcomes. Our Care Coordinators are the backbone of the organization, working directly with patients and our Medication Management Teams. Care Coordinators work from home and have the primary responsibility of contacting patients to encourage and facilitate medication adherence. Competent handling of Protected Health Information and maintaining compliance with the Health Insurance Portability and Accountability Act (HIPAA) are essential. Care Coordinators will also become versed in navigating health insurance formularies, pharmacy benefits, and Medication Therapy Management.
Duties:
- Carefully maintain Protected Health Information in compliance with HIPAA standards
- Make outbound phone calls to encourage adherence to prescribed medications
- Build trust with patients while describing the benefits of our clinical pharmacy and care management services
- Coordinate calendar availability with the Bridgewell care team
- Send follow up documentation to patients and doctors
- Coordinate with doctors to establish optimal care plans for patients
- Complete follow up phone calls with patients to ensure they are adhering to their care plan
- Work with health insurance companies to navigate pharmacy and medical benefits
- Maintain internal documentation to support Bridgewell operational goals
- Document patient contact and care plans in the electronic medical record
Skills:
- Friendly & outgoing with great communication skills and a proven ability to relate with the senior population
- A self-starter, capable of working independently without onsite supervision
- Great attention to detail
- Professional demeanor and ability to represent company standards with customers
- Ability to easily multi-task between different computer systems
- Excellent typing & documentation skills
- Technologically adept with a proven ability to utilize and problem solve varying software applications
- Resilience in difficult situations, including when patients decline our services or exhibit anger
- Grit: we are looking for individuals who pride themselves on overcoming all obstacles to take care of the customer and complete the job.
- Basic knowledge of prescription drug names and disease states
- Experience working in a healthcare setting

Note: We are seeking reliable, extremely self-motivated individuals for 100% remote positions. This role offers maximum control over your hours, with no corporate benefits provided. Employer provides all necessary hardware (computer/peripherals) shipped directly to your home at no cost to you.
Mandatory Requirements:
- Dedicated Workspace: Must have a secure, private, and quiet home office environment.
- Internet Speed: Stable, high-speed internet connectivity is required.
- Connection Type: Fiber internet connection is strongly preferred.
- Availability: Must commit to 20-40 hours/week
Pay: $18.00 - $23.00 per hour
